What are Chronic Health Conditions?

Chronic Health Conditions is a large category of disability and disease - the CDC's definition broadly includes any condition that lasts 1 year or longer and requires on-going medical attention or limits to daily living activities (taken from https://www.cdc.gov/chronicdisease/about/index.htm). Some of the biggest chronic illness in the United States are heart disease, cancer, and diabetes, but many others fall into this category, such as allergies, arthritis, asthma, and other types of physical and mental impairments.
